2003 Board of Directors Membership Roster. This document lists the 2003 BVCA Partnership board of directors by county and sector represented. It also notes current terms.

2003 Board of Directors Standing Committees. This document lists the 2003 BVCA Partnership board committees and their members.

Board of Directors Annual Calendar. This document shows items the board of directors considers annually and the month those items will appear on the agenda.

Board of Directors Nomination Form. Use this form to recommend individuals to serve on the board of directors. Staff will contact your recommendations to explain more about BVCA Partnership and director roles and responsibilities.

Board of Directors Private Sector Nomination Form. Use this form to let BVCA Partnership know that your business or organization has formally nominated someone to represent them on the board of directors.

BVCA Partnership Mileage Reimbursement Forms. These forms are used by volunteers and members of the Board of Directors who are requesting mileage reimbursement. Please check with your staff contact person to see if you are eligible for reimbursement. That person will assist you in completing the correct form and tell you how to ensure your request is properly processed.

Forms

BVCA Partnership Forms

BVCA Partnership Basic Intake Form (BIF), Member Basic Intake Form, and Instructions. These forms are completed by all program participants, regardless of the service(s) they request. If you are planning to apply for services, you may want to download, print and complete a hard copy version of this form to save yourself enrollment time. If you are unable to complete the BIF prior to your appointment, staff will gladly assist you in filling it out at that time.

BVCA Partnership Board of Directors Nomination Form. Use this form to recommend individuals to serve on the board of directors. Staff will contact your recommendations to explain more about BVCA Partnership and director roles and responsibilities.

BVCA Partnership Board of Directors Private Sector Nomination Form. Use this form to let BVCA Partnership know that your business or organization has formally nominated someone to represent them on the board of directors.

BVCA Partnership Donation Forms. For your convenience, Blue Valley Community Action Partnership has two forms to use when donating goods and services. The basic Donation Record is used when donating goods and/or volunteering on an occasional basis. If you regularly volunteer more than 10 hours per week, please use the Volunteer Time Record. BVCA Partnership is extremely proud of the local in-kind support we receive. We encourage you to volunteer and work with Blue Valley Community Action Partnership's many programs.

BVCA Partnership Mileage Reimbursement Forms. These forms are used by volunteers and members of the Board of Directors who are requesting mileage reimbursement. Please check with your staff contact person to see if you are eligible for reimbursement. That person will assist you in completing the correct form and tell you how to ensure your request is properly processed.

Partner Forms

The following forms are used to apply to programs operated by our partners. They are provided here as a convenience to our users. Because these forms can be updated without notifying us, we recommend you call our home office (402.729.2278) to be certain you have the most recent version of a form.

Every Woman Matters. Every Woman Matters is a state-subsidized health program providing free or reduced cost screenings for breast (mammogram) and cervical cancer (PAP test). This application form is 720K. For assistance in completing this form, please call our home office at 402.729.2278.

Kid's Connection Application. Kid's Connection is a state-subsidized health insurance program for moderate-income children. For assistance in completing this application, please call our home office at 402.729.2278.

Respite Subsidy Application. The Nebraska Respite Network provides life-span respite services in southeast Nebraska. While there are no income guidelines to receive information and referrals, subsidies are only available to low-income families. For assistance in completing this application, please contact us at 402.729.2278.
