The REACH program is designed to address disparities in chronic disease outcomes among low-income rural populations. Through patient-centered education, early identification of health risks, and streamlined referral pathways, REACH connects individuals to medical care, nutrition services, and community-based resources. The program emphasizes accessibility, care coordination, and culturally responsive support to reduce barriers and improve long-term health outcomes.
The REACH program was designed to assist individuals and families with a low income; however, anyone in need of the service may apply. To participate, please contact your local BVCA center.

Telehealth services are for individuals who are not comfortable with technology or for those that simply do not have access to a computer, smart phone or internet but still need to connect to our services. While BVCA Partnership is focusing these services for elderly, homeless, and/or low-income individuals, our Telehealth services are available to anyone who has a need.
Medicaid Vaccines for Children (VFC) and Private Vaccines are offered in Fillmore, Jefferson, Saline and Seward Counties. All clients must make an appointment and have their insurance eligibility confirmed prior to receiving any vaccines. Walk-ins are NOT accepted at this time. Please bring your insurance card to each visit.
WIC is the nation’s most successful public health nutrition program. We provide healthy food, nutrition education, and community support to income-eligible pregnant women, mothers of infants, and children up to 5 years old.
The Breastfeeding Peer Counseling Program (BFPC) provides education and support for pregnant and postpartum women to ensure a successful breast-feeding experience. The BFPC program is based on the FNS WIC Breastfeeding Model for Peer Counseling (formerly called Loving Support© Model for Peer Counseling).
Fillmore County Rural Transit Services is a county-wide transportation service within and out-of-the-county. it is available for a small fee for general public, with priority given to elderly and handicapped residents.
Commodity Supplemental Food Program (CSFP) provides nutritious supplemental food from USDA to seniors 60 years of age and older. Participants must also meet income guidelines.
We distribute donated perishable and non-perishable food, household items, and paper products, distributed to families and individuals in emergency situations. Several centers receive donations of USDA food from the Lincoln Food Bank.
